Here is a quick guide to

ONLINE COLLEGE ADMISSION APPLICATION

The application for the College Admission for 1st Semester SY 2020-2021 is now online. To all aspiring applicants, here are the important notes you need to remember:

1. Application Period

Online Application for Admission will start on July 8, 2020 up until July 10, 2020 only.

2. Entrance Examination Schedule

All applicants for Admission will be scheduled for their entrance examination from July 13, 2020 up until July 17, 2020 only.

*No Online Application, No Entrance Examination

ADMISSION PROCEDURES

Step 1. Online Application for Admission

MANDATORY REQUIREMENTS

- An active google mail (gmail) address must be provided. Your google mail address will be used to send the links pertaining to your application.

- To avoid delay and disapproval, soft copy / scanned copy of 2x2 picture in plain white background wearing decent attire with exact dimensions of 300 pixels by 300 pixels will be used for the profile picture upload. These are strictly required to be able to process your application.

Step 2. Entrance Examination

Examination links will be given through email. Kindly check your provided email address for updates on your selected examination schedule.

Note: Examination schedules will be posted on our official facebook page.

Step 3. Qualified Application

List of qualified applicants will be posted on our official facebook page.

Step 4. Confirmation of Application

Kindly check your provided email address for updates. An email will be sent to each and every qualified applicants to confirm your application.

Step 5. Cumulative Record and Screening

Kindly fill up the Student Cumulative Record, Needs Assessment Survey, and Admission Interview Sheet for our preference.

Qualified applicants will be screened via Zoom or through phone interview by the Deans and Department Chairs of their respective schools.

Step 6. Certificate of Admission

All successful applicants will be given an e-certificate of admission and will proceed to the enrolment process.

Note; All requirements must be submitted as soon as face to face sessions resume.

SENIOR HIGH GRADUATE

- Grade 12 Report Card

- Good Moral Certificate

- Form 137 (Pupil’s Permanent Record)

- PSA Birth Certificate

- Barangay Clearance

TRANSFEREES

- Transcript of Records

- Good Moral Certificate

- Honorable Dismissal / Transfer Credentials

- PSA Birth Certificate

- Barangay Clearance

ALTERNATIVE LEARNING SYSTEM PASSER

- Must be an ALS PASSER of 2018 or lower

- Certificate of Rating

- Good Moral Certificate

- PSA Birth Certificate

- Barangay Clearance

OLD CURRICULUM (HIGH SCHOOL GRADUATE)

- Form 138 (High School Report Card)

- Good Moral Certificate

- Form 137 (Pupil’s Permanent Record)

- PSA Birth Certificate

- Barangay Clearance

OTHER REQUIREMENTS

All qualified Applicants must provide the following:

-Long white Folder with Fastener

-Three 2x2 pictures

-Three 1x1 pictures
